Personalized Welcome Letter & Referral ID Loyalty Card - This letter introduces patients to the HealthMatters™ program with a friendly welcome message from your agency. Included in the letter is a plastic Referral ID Loyalty Card. The wallet-sized card is personalized for each patient and customized with your logo so that patients remember to choose your agency when needing home care services. The card also provides pharmacy discounts on prescription drugs, over-the-counter medications, diabetic equipment, hearing aids, durable medical equipment, and more!. |

FAQ Flyer - This flyer — customized with your logo and contact information — answers general questions about home health services, benefits, and payments, as well as informs patients of the specific services your agency provides. |

Pharmacy Flyer - This flyer explains the pharmacy discount program for HealthMatters™ members. There are six savings programs available to members — each described in detail in the flyer — along with a list of participating retail pharmacies. The flyer also describes the four easy ways members can take advantage of these programs at their local pharmacy, by mail, online, and by phone. |

My Health Log - This log booklet provides patients with a convenient way to keep track of at least three months of health records, including:
- Medicines
- Doctors Appointments
- Immunizations
- Cholesterol
- Surgeries & Procedures
- Blood Sugar
While the log booklet is sent to HealthMatters™ members as part of the Platinum Plan, it also can be used as a stand-alone product, as in the Basic Plan. |

Personalized Holiday Calendar Card - Sent at Christmas time, these cards deliver a friendly holiday greeting. The gift of a calendar for the upcoming year — designed with your logo — is printed inside the cards to remind patients of your agency all year long. |
